Healthcare Partnerships Manager
Your Healthcare Partnerships Manager is a professional who will work with hospital executives to set up local or even national partnerships, and address their needs for a successful referral program.
Community Relations Advisors (CRAs)
The Community Relations Advisors work directly with your team. They act as the liaison between the discharge team, and/or family and our Senior Living Advisors.
Senior Living Advisors (SLAs)
The Senior Living Advisors are experts on the local senior living landscape and they work with individual families to help them with all aspects of senior care and housing.
Data analysts will be providing insights on transition performance.
